Preparing for your first in-person appointment:
Please schedule online using the Book Now tab on the top left-hand corner of this page, or call the office to book an appointment.
Expect your first appointment to last about 45 minutes - 1 hour.
Please bring the following:
- Shoes that you wear on a day-to-day basis as well as shoes you wear for exercise (running, cycling, dancing, hiking, tennis etc.) to your first appointment.
- Correct Toes (if applicable)
- Orthotics and/or footbeds you wear in your shoes (if applicable)
- X-rays of MRIs of the foot in the past 3 years (if applicalbe). Please have the hard copy of the images (on a disc) and the report sent to our office ahead of time, or bring them to your appointment.
Remote Consultation Overview:
Remote consultations offer a great platform to get more specific medical questions answered, gather more information on how we approach a variety of other foot problems, and receive an individualized treatment plan.
Step 2: Once all information has been submitted, we will reach out to you to schedule, and to gather your payment information. Please note we do not accept insurance.
Step 3: On the date/time of your consultation, the physician will contact you directly via your preferred method of contact (Skype, Facetime, or Phone).
After your consultation you will be provided with a personalized treatment plan, additional support resources, and the reciepts from your consultation.
We are out-of-network for all insurance companies; we do not bill insurance. Payment is due at the time of a visit for all services.
We are not able to see patients who have OHP, Care oregon, HealthShare Oregon, Family Share, Medicaid, or those insured under worker's compensation or auto accidents.
For those patients that we can see, we do provide each patient with the necessary information needed to bill their own insurance in order to be reimbursed at the out-of-network rate. This rate is based on your individual plan. Call your insurance provider to find out the details of your plan.
*Please note that Medicare, Tricare and Kaiser do not reimburse patients for visits here at our clinic.
Procedures, foot/ankle tapings, injections and/or products purchased are billed separate from the office visit fee. Treatments such as injections, foot/ankle tapings or toenail procedures and products purchased (socks, pads, Correct Toes, shoes) are not included in the office visit fee.
