Insurance Policies

Northwest Foot & Ankle is out of network for all insurance providers, meaning we do not accept or bill insurance. 

We can provide receipts and documentation, so patients can submit to insurance on their own. Submitting to insurance is optional, and the patient’s responsibility. We cannot guarantee reimbursement from your insurance; please consult with your insurance provider for more questions about seeing an out-of-network physician.

Patients insured by Medicare or Medicaid (OHP or affiliate plan) will NOT be reimbursed for visits. These patients will need to sign a waiver stating they understand this and will not attempt to submit for reimbursement.

We cannot see patients who are insured by Tricare or those who have Worker’s Compensation or Auto Accident Coverage. We welcome you to call our office to learn about other natural treatment referral options.

Medical Appointments

  • New patient appointment: $220-380
  • Return patient appointment: $130-270

An appointment deposit will be collected at the time of booking:

  • New patient visit: $150
  • Return visit: $75

*Specialty medical procedures and purchases of any over-the-counter products are an additional cost. 

Please note: due to Oregon law, patients who have not been seen by a physician at NWFA in the past three (3) years must be re-admitted as a “New Patient.”

Remote Consultations (Skype, Facetime, Phone)

  • New patient remote consultation: $200
  • Return patient remote consultation: $150

An appointment deposit will be collected at the time of booking:

  • Remote consultation: $150
  • Return remote consultation: $75

*Patients cannot submit to insurance for remote consultations.
